Tax, PACE Forms Available at Sen. White’s Franklin Office
Harrisburg – Sen. Mary Jo White (R-21) has 1998 Pennsylvania tax forms
and schedules, 1998 property tax/rent rebate forms, and PACE prescription
drug plan forms available at her Franklin district office.
The office is located at 1256 Liberty St., Suite B., in Franklin. Office hours are Monday through Friday, 9 a.m. to 4:30 p.m.

In addition to standard tax forms, constituents can obtain forms for
two programs geared mainly toward senior citizens: Property Tax/Rent Rebate
and the PACE/PACENET prescription drug plan.
Residents age 65 years or older and married to someone 65 years of age
or older; or age 50 or older and a widow/widower; or age 18 or older and
permanently disabled may be eligible for up to $500 in property tax or
rent rebate benefits.
Total household income must be less than $15,000 annually to be eligible.
Under PACE, single people age 65 and older with an annual income under
$14,000, and couples age 65 and older with a $17,200 combined income pay
$6 for prescription medicine.
Under PACENET, single seniors with an income between $14,000 and $16,000,
and couples with incomes between $17,200 and $19,200 are eligible for benefits
after reaching a $500 deductible per cardholder.
PACENET participants pay $8 for generic medicine and $15 for name-brand
prescriptions.
